首发精神分裂症患者前额叶皮质背外侧-腹内侧T2信号强度与外周血白细胞的研究

【摘要】 目的:探讨首发精神分裂症患者前额叶皮质背外侧(DLPFC)-腹内侧(VMPFC)的T2信号强度与外周血白细胞(WBC)水平的关系。方法:35例首发精神分裂症患者(患者组)与35名健康志愿者(对照组)均接受3.0 T磁共振(MRI)头部扫描与血液采集,以获取DLPFC、VMPFC的T2信号值及血液白细胞(WBC)总数值,并进行比较;同时患者进行阳性与阴性症状量表(PANSS)评分;分析患者DLPFC、VMPFC的T2信号值、WBC总数及PANSS评分间的相关性。结果:患者组两侧DLPFC的T2信号强度、WBC总数明显高于对照组(t=2.57,2.35,2.16;P均<0.05)。患者组两侧DLPFC的T2信号强度与WBC总数呈正相关(r=0.21,0.36,P均<0.05);两侧DLPFC的T2信号强度、WBC总数与PANSS阴性症状分呈正相关(r=0.38,0.35,0.41;P均<0.05)。结论:首发精神分裂症患者DLPFC的T2信号强度与外周血WBC水平异常,提示炎症反应是精神分裂症发病的机制之一。

【Abstract】 Objective:To explore the relationship between the T2 signal intensity of dorsolateral(DLPFC)-ventromedial(VMPFC) prefrontal cortex and the level of peripheral white blood cells(WBC) in patients with first-episode schizophrenia. Method:35 patients with first-episode schizophrenia(patient group) and 35 healthy volunteers(control group) received 3.0 T magnetic resonance imaging(MRI) head scans and blood collection to obtain the T2 signal intensity of DLPFC,VMPFC and WBC counts which were compared between the two groups.Positive and Negative Symptom Scale(PANSS) scores were also assessed.The correlation between the T2 signal intensity of DLPFC and VMPFC,the WBC counts and PANSS scores were analyzed. Results:The T2 signal intensity of the bilateral DLPFC and WBC counts in the patient group were significantly higher than those in the control group(t=2.57,2.35,2.16;all P<0.05).In the patient group, the T2 signal intensity of the bilateral DLPFC were positively correlated with the WBC counts(r=0.21,0.36;both P<0.05).In addition, the T2 signal intensity of the bilateral DLPFC and WBC counts were positively correlated with the negative symptom scores(r=0.38,0.35,0.41;all P<0.05). Conclusion:The abnormal T2 signal intensity of DLPFC and peripheral WBC level in the patients with first-episode schizophrenia may indicate that inflammatory response is one of the pathogenesis of schizophrenia.
